New Patient:
If you're a new patient, follow these steps:
· Click the New Patient? link and follow the instructions to set up your account.
· Once you set up yor account, log in to your account, complete all required information, and submit your appointment request.
Current Patient:
If you’re an existing Southern California Medical Center patient, please follow these steps:
· To log in and request an appointment, you must verify your identity. Click "Current Health Center Patient" and follow the steps to receive a verification code via text and email.
· If you are a current patient and do not have a username and password yet, click "Don’t Have Username and Password?" to set up your account.
